• To call the patients who have accepted to be enrolled in the Patient Support and enroll them by taking informed consent.
• Create the patient profile by administering questionnaire over the telephone.
• Provide customized counseling to the patient on Diabetes and its management for appropriate behavior modification.
• Provide expect advice on Life Style modification and Diet.
• Should have the ability to develop good relationship with the patients.
• Maintaining Data after every counseling session with the patient.
